Connecting Kentucky Families to the Support They Need
Targeted Case Management (TCM) services help children with disabilities and their families connect to the care, services, and community resources they need to improve daily functioning, independence, and overall quality of life.
Our remote TCM team works closely with families, healthcare providers, schools, and community agencies throughout Kentucky, serving the entire state through phone, video, and electronic communication. No matter where you are in Kentucky, our case managers can help.
Targeted Case Management is available statewide in Kentucky through remote service delivery.
What Does a Case Manager Do?
Your case manager serves as your guide and advocate, working across systems to make sure your child and family have access to the services, resources, and support that matter most. TCM services focus on:
- Coordinating medical, behavioral health, therapy, and educational services
- Locating community resources and support programs
- Helping families apply for benefits, waiver programs, and assistance services
- Advocating for children within school systems, healthcare settings, and community programs
- Developing individualized care plans based on each child’s strengths and needs
- Monitoring progress and adjusting plans as needs change
- Providing caregiver education, guidance, and ongoing support
- Coordinating scheduling and communication between providers
- Addressing barriers to care including transportation, financial challenges, and access to services
- Connecting families with local and statewide disability resources
What Can TCM Help With?
Case managers help families navigate a wide range of needs beyond clinical care, including:
- Access to Already Autism Health's ABA therapy or diagnostic assessment services
- Transportation needs
- Medical and dental care
- Mental health or co-occurring condition support
- Food insecurity
- Housing
- Other individual or family needs
Who Is Eligible for TCM?
Children and adolescents in Kentucky may qualify for Targeted Case Management if they:
- Have a documented disability, developmental delay, behavioral health condition, or chronic medical need
- Require assistance accessing healthcare, educational, therapeutic, or community-based services
- Experience barriers related to transportation, financial hardship, communication challenges, or limited access to local resources
- Need ongoing coordination between multiple providers or agencies
- Are enrolled in Kentucky Medicaid

Insurance & Payment
Michelle P. Waiver (MPW) and Supports for Community Living (SCL) services are funded through Kentucky Medicaid for eligible participants and do not involve out-of-pocket costs
Our team also verifies eligibility and obtains the necessary authorizations before services begin.
